Three month after delivery a 20-year-old female further evaluated about pelvic pain during & after delivery.

Imaging Procedures and Findings

A 20-year-old woman with chronic pelvic pain during and after delivery evaluated by CT scan. In performed study, a soft tissue mass (about 74*50mm) depicted in front of uterus which showed non enhancing center also enhancing periphery the same as uterus. Simultaneous U/S study confirmed degenerated subserosal fibroid also intact ovaries.

 

Discussion

Cystic adnexal mass is a differential diagnosis but uterus is separate from mass. Leiomyosarcoma always contains areas of hemorrhage and necrosis and overlaps in imaging features with degenerated leiomyoma. Pelvic or tubo-ovarian abscess is a complex cystic adnexal mass inseparable from ovary. Most degenerating leiomyomas are asymptomatic. Risk for pain increases with size and is high in lesions that are larger than 5 cm in diameter.

 

Final Diagnosis

Degenerated fibroid
